POSITION Overview:
Our Home Care Managers (HCM) provide ongoing evaluation and monitoring of our client base to ensure that the in-home services provided continue to meet the needs of the individual.
HCMs continually assess and reevaluate client needs on an individual basis to provide them and their caregivers with linkage to an extensive assortment of information and assistance that can improve quality of life in a home and community based setting.
As needs evolve, HCMs assist individuals in exploring options and making necessary changes to their plan of care.
EDUCATION:
We are looking for:
a Associates degree in nursing; or An Associates degree in any field with a minimum of four year full-time, direct service experience with the elderly or disabled (this experience includes assessment, care plan development, and monitoring).
a Registered Nurse with one years experience in human services -- or -- a Bachelors degree in Social Work, Psychology, Counseling, Gerontology, Nursing or Health and Human Services -- or -- a Bachelors degree in any field with a minimum of two years full-time, direct service experience with the elderly or disabled -- or -- a Masters Degree in Social Work, Psychology, Counseling, Gerontology, Nursing or Health Services may substitute for the required minimum of two full time direct services experience AREA:
The main office is located in Dillsboro, Indiana.
Our service area includes Dearborn, Ohio, Switzerland, Jefferson, and Ripley Counties.
TRAINING:
You will begin developing hands-on experience within your 1st week by shadowing veteran HCMs and by working collaboratively with your Supervisor to allow the observation of the ins-and-outs of the HCM position from office culture to home visits and data entry.
